If you want to plan for economical disposition of your body after death — a simple but dignified cremation or burial — join the Memorial Society of Georgia. A lifetime membership costs only $35 and gives the peace of mind of making your own plans now at an assured low and reasonable cost, and sharing them with those who live after you.
To become a member, submit the online application below. You can also print the download version of the application and send it in by mail or email. If you are unable to submit online or download the application, call 404-634-2896 to request that an application be mailed to you. Each adult member requires a separate application. Children under 18 years old are covered at no charge when a parent joins.
When you join, you will be required to select one of our Funeral Home Partners. See the list by clicking HERE. Note that the Funeral Directors designated by asterisks are preferred providers, meaning they offer all Memorial Society plan options at the lowest contract rate. The rest of the listed Funeral Directors offer only the “Simple Direct Cremation” and “Simple Direct Burial” options at Memorial Society reduced pricing, and may charge substantially higher for plans other than Simple Direct Cremation or Burial. Your records will be kept on file with the Memorial Society and also with the Funeral Director that you select. After joining, you may change your selection at any time.

As soon as we receive your application, we will send you some other documents for your use. One of them is a form that requests the information needed by the Funeral Director to fill out the death certificate. Please note that payment is NOT expected for the cremation or burial until time of death. (Please see our Frequently Asked Questions page for more info.)
