Please print this out for your records and give a copy to your family members and to the person responsible for your final disposition.
The Memorial Society of Georgia will send a copy of your Pre-Arrangement Form to your chosen funeral director. You will receive the second half of the Pre-Arrangement Form via email if you provided an email address. If you don’t see it, please be sure to check your spam folder. This second half contains important information for your chosen funeral director.
